Such criticisms have however been convincingly critiqued in their own right by scholars examining the popular communication of scientific and medical research (Nelkin 1996, Gregory and Miller 1998, Seale 2002, Schudson 2003).

They critique how RCA can lead to organizational learning, through investigating the cause(s) of incidents, by looking at how the use of RCA was played out in context. Finally, Iedema examines the influence of changing contexts and dynamics in health care, and argues for a real-time, in situ understanding of and response to medical errors, harms and bad practice. He calls for a new knowledge to be developed, incorporating formal knowledge and awareness of in situ complexities. Taken as a whole, the book advances a strong, coherent argument that is much needed to counter some of the uncritical assumptions that need to be described and analysed if patient safety is, indeed, to be achieved.

Her research focuses on teamwork, management and work motivation. Justin Waring is Professor of Health Systems and Policy at Nottingham University Business School. His research seeks to develop theoretical and methodological synthesis across social science disciplines to better understand clinical risk and organizational learning. This includes a study on the implementation of incident reporting, an ethnographic study of the threats to patient safety in the operating theatre, a real-time study of accident investigation, an evaluation of electronic prescribing in primary care and a mixed-methods study of learning across organizational and occupational boundaries in new clinical settings.
